We are US based looking to outsource our requirement of Software Development Service. We are looking for development of software solution for patient web portal.
Scope:
1. Ability to manage a larger patient population without an increase in staff
2. Increase patient involvement in their own care
3. Must have secure web based access
4. Solution must be universally accessible by patients regardless of disability or socioeconomic status
5. Must support medical home model with online management of chronic disease patients.
